How to make form with two columns (table)?

I want to separate the fields inside the form into two columns. The left column consist of Employee ID until Leave Type and the right column consist of Email until Note. The submit button will be at the middle bottom.

Employee ID ------------ Email
Name -------------------- Tel
Password---- ------------ Extension
Re enter Password ----- Fax
Department ------------ Mobile
Group ------------------- Commencing Date
Employment Status----- Note
Leave Type-------------

Submit (button)

I also attach the codes I did for the form. I just don't quite understand decorations. Please help.

I would say you can get a 2 columns form using fieldset and css in your case.
